If you are a new patient we advise that you arrive 10-15 minutes before your appointment time in order to register as a new patient.

 

If you'd like to save time on the day of your appointment, please print out the New Patient Information Form below, fill in your information, and bring it with you to your first appointment.

Please note we cannot accept the form electronically due to patient confidentiality.

 

You can also print and sign our Doctor Patient Arrangement Sheet which gives us the authority to use ICD10 codes on your behalf when claiming from Medical Aid.
